「新式外來人口統一證號」變更提醒事項 Notes for New UI No. for Foreign Nationals

Dear Customers,

感謝您長久以來的支持，為因應「內政部推動外來人口新式統一證號換發政策」，若您已換發新式統一證號者，提醒您下列事項：
Thank you for your support to Cathay Century Insurance. In response to the "Ministry of the Interior´s Policy of Promoting the Issuance of the New UI Number for the Foreign Nationals", if you have already been issued a new UI Number, you are reminded of the following:

資料變更說明：
1. 請您本人攜帶換發之「最新身分證件(居留證)」正本，親臨本公司任一分公司辦理變更統一證號作業事宜。
Please bring the original of your "latest identity certificate (Residence Permit)" to any branches of Cathay Century Insurance to update your UI Number.

2. 您也可連絡您的業務員，提供批改申請書並親簽，以及「最新身分證件(居留證)」正反面影本，由業務員代為申請。
You can also contact your salesperson and provide the correction application form which has your personal signature, as well as a photocopy of the front and back of your "latest identity document (Residence Permit)".

3. 若您完成證號變更後，建議您洽銀行同步變更您的統一證號，才能順利完成您與該單位約定之入/扣款作業。
After you have completed changing your UI Number, we recommend that you update your new UI Number with the bank so that we can complete smoothly the payment deposit/deduction operations.
